Research on health effects of secondhand exposure to e-cigarettes is still emerging. The quantity of these carcinogens is less that what is found in conventional cigarettes. While some young people may be able to quit e-cigarette use on their own, others, particularly daily users, are likely to find this to be very difficult.

Using an e-cigarette is often called "vaping" or "JUULing." 2Most e-cigarette users do not consider themselves to be smokers. E-cigarettes, vapes, vape or hookah pens, e-pipes, and other vaping products are battery-powered devices that allow users to inhale, or vape, aerosolized liquid (e-juice).
Nicotine yields for different e-cigarette brands have been reported from 2 to 313 µg puff−1 while for conventional cigarettes smoke the values ranged from 170 to 232 µg puff−16,31,36,48,64. Nicotine poisoning refers to the toxic effects of consuming nicotine, a chemical in all tobacco products. A recent increase in poisonings is due to liquid nicotine, a product in the popular e-cigarette. Symptoms include vomiting, rapid heart rate, unsteadiness and increased salivation. The comprehensive review of the evidence finds that almost all of the 2.6 million adults using e-cigarettes in Great Britain are current or ex-smokers, most of whom are using the devices to help them quit smoking or to prevent them going back to cigarettes. It also provides reassurance that very few adults and young people who have never smoked are becoming regular e-cigarette users (less than 1% in each group).
Given that nicotine easily diffuses from the dermis to the bloodstream, acute nicotine exposure by e-liquid spilling (5 mL of a 20 mg/mL nicotine-containing refill is equivalent to 100 mg of nicotine) can easily be toxic or even deadly [8]. Thus, devices with rechargeable refills are another issue of concern with e-cigarettes, especially when e-liquids are not sold in child-safe containers, increasing the risk of spilling, swallowing or breathing. An E-cigarette is any device or delivery system sold in combination with nicotine for a single price which can be used to deliver to a person nicotine in aerosolized or vaporized form. An E-cigarettes may also be known as e-cigar, e-pipe, vape pen, or e-hookah or other names. The definition of E-cigarettes includes any liquid or substance containing nicotine, whether sold separately or sold in combination with any device that could be used to deliver to a person nicotine in aerosolized or vaporized form. For each 0.7 milliliters of e-cigarette e-liquid not sold due to these policies, the authors calculated that 15 additional cigarettes were purchased.
Due to what has been called an ‘epidemic’ of youth use of these products, FDA has prioritized prevention efforts. The agency has taken a multitude of actions to keep ENDS out of the hands of youth, from policy making to enforcement to education. While e-cigarettes can generally be a lower-risk alternative for adults who smoke cigarettes, the use of e-cigarettes is not risk-free. These products deliver harmful chemicals and contain nicotine, which is highly addictive. Moreover, given the harmful chemicals found in e-cigarettes, further high-quality research on both short- and long-term health outcomes is needed. The longer ENDS and other e-cigarettes are on the market, the more information we know about their impacts on health.

The harmful effects of CS and their deleterious consequences are both well recognised and widely investigated. However, and based on the studies carried out so far, it seems that e-cigarette consumption is less toxic than tobacco smoking. This does not necessarily mean, however, that e-cigarettes are free from hazardous effects. Indeed, studies investigating their long-term effects on human health are urgently required. In this regard, the main additional studies needed in this field are summarized in Table 3.

E-cigarettes are hooking a new generation on nicotine – putting millions of kids at risk and threatening decades of progress in reducing youth tobacco use. During the Vietnam War, cigarettes were included with C-ration meals. In 1975, the U.S. government stopped putting cigarettes in military rations. During the second half of the 20th century, the adverse health effects of tobacco smoking started to become widely known and printed health warnings became common on cigarette packets. This health advisory is being issued by the State Health Officer to inform the public about the alarming statistics on current e-cigarette use among youth in Wisconsin. In Wisconsin, current e-cigarette use among Wisconsin high school students increased 154% between 2014 and 2018.
The 2019 outbreak of EVALI cases emerged against a backdrop of an ongoing vaping epidemic among youth. "These products appear to be designed to circumvent the laws and regulations in place to protect people—especially children—from the harmful effects of smoking and tobacco use," Jordt said. The CDC advises people who have asthma to avoid tobacco smoke, yet Minnesota teens who have asthma are more likely to breathe someone else’s tobacco smoke or e-cigarette aerosol than teens who do not have asthma. Minnesota’s teens who have asthma are less likely to be protected by rules against smoking in the family’s homes or vehicles. Individuals included from the PATH study were 21 years and older daily cigarette smokers and reported any use of e-cigarettes within the previous 30 days. The new study findings follow on the heels of a report from the Substance Abuse and Mental Health Services Administration (SAMHSA) showing growing numbers of e-cigarette users.
As such, it is imperative that e-cigarette cessation programs focused on adolescents are developed, evaluated, and implemented," Halpern-Felsher wrote. "This may point to the changing norms around e-cigarette use and the fact that quitting vaping among young people is becoming more normative and accepted," she said. Most of the study participants, about 87%, reported that they had tried to quit in the previous year, and about 94% reported feeling somewhat or very addicted to vaping. E-cigarettes usage by adolescents has become an epidemic according to the American Lung Association–20% (5 million) of all youth use e-cigarettes, a 135% increase in just two years.

In 2019, four years after JUUL introduced a new nicotine delivery system coupled with a relentless online, youth-focused marketing campaign, more than a quarter (27.5%) of high school students reported using e-cigarettes. Short-term studies haven't found evidence that exposure to secondhand aerosol from e-cigarettes hurts lung function, with one notable exception. Researchers found that people who were around vaping aerosol showed increases in serum cotinine, which is a marker that someone was exposed to nicotine (an ingredient often found in e-cigarettes). The majority of cases reported to poison centers are due to accidental exposures in children less than 6 years old. This reflects the increasing availability of these products in the home due to use by guardians or older siblings. Additionally, in the last 5 years, the FDA has warned companies selling e-cigarette devices made in the shape of toys, foods or cartoons that specifically appeal to use by young people. Of this sample, 3,242 participants developed heart failure within a median follow-up time of 45 months. As e-cigarettes deliver flavorings, additives, and, typically, nicotine through an inhaled aerosol, they are harmful to both the user and those who are exposed secondhand.

Making small changes to your vape products or how you vape should help. Side effects are usually easily managed and should not stop you from vaping as a way to quit smoking. You're roughly twice as likely to quit smoking if you use a nicotine vape compared with other nicotine replacement products, like patches or gum.

But when it comes to your heart health, a pair of recent studies show e-cigarettes are just as dangerous – and possibly are even more dangerous – than traditional cigarettes. The long-term effects of smoking cigarettes are well-documented and include an increased risk of stroke, heart disease, and lung cancer.
Nicotine use can have an impact on learning, memory and attention and increase the likelihood of addiction to other drugs in the future. While some people have reported that using e-cigarettes has helped them quit smoking, there are other proven, safe and effective options that should be explored first. That figure is controversial and might be a little high, says Kenneth Warner, a tobacco policy researcher at the University of Michigan. But, he adds, "The worst critics of e-cigarettes would probably argue they're a half to two-thirds less dangerous. But from a practical view, they're probably on the order of 80% to 85% less dangerous, at least." A 2015 expert review from Public Health England estimated e-cigs are 95% less harmful than the real thing.
CATCH My Breath’s curriculum attempts to empower students with this information for them to decide whether to pick up an e-cigarette, Marcella Bianco, the program’s director of government partnerships, explained. Once the school year starts, the 34 middle and high schools in the district join 56 others across the state that have already implemented the prevention program since its launch in 2017. The participants were encouraged to set a quit date and were asked about their vaping behavior via follow-up text message assessments. The trial participants were recruited via ads on social media, including Instagram, Facebook and Snapchat. From October 2021 to October 2023, about half of the participants were randomly assigned to the This is Quitting text message program.
A 2018 report from the National Academies Press (NAP) found significant evidence that taking a puff from a nicotine e-cigarette triggers an increase in heart rate. In summary, it seems that either smoking or nicotine vaping may adversely impact on COVID-19 outcome. However, additional follow up studies are required in COVID-19 pandemic to clarify the effect of e-cigarette use on lung and cardiovascular complications derived from SARS-CoV-2 infection. It is also noteworthy that among the 3 most cytotoxic vapours for HUVEC evaluated in the Putzhammer et al. study, 2 were nicotine-free, which suggests that nicotine is not the only hazardous component in e-cigarettes [24]. However, while vape products can reduce the amount of tar and other chemicals a person inhales, they can increase a person’s nicotine dependency. The National Institute on Drug Abuse (NIDA) states that around half of all smokers try to quit every year, while only 6% manage to do so.
According to the 2021 National Youth Tobacco Survey, more than 2 million U.S. middle and high school students reported using e-cigarettes in 2021, with more than 8 in 10 of those youth using flavored e-cigarettes. The mid-to-long-term consequences of e-cigarettes are not yet known, as it's a new product and has been sold for less than a decade in the U.S. While much remains to be determined about these lasting health consequences of these products, we are very troubled by what we see so far. The inhalation of harmful chemicals can cause irreversible lung damage and lung diseases.

Use of microporous ceramic as a wicking material improves heating efficiency, but how it affects the chemical emissions of these devices is unclear. We assessed the emissions of a pod e-cigarette with innovative ceramic wick-based technology and two flavoured e-liquids containing nicotine lactate and nicotine benzoate (57 and 18 mg mL−1 nicotine, respectively). Among the studied harmful and potentially harmful constituents (HPHCs) listed by the US FDA and/or WHO TobReg, only 5 (acetone, acetaldehyde, formaldehyde, naphthalene and nornicotine) were quantified at levels of 0.14 to 100 ng puff−1. In the combustible cigarette (Kentucky reference 1R6F), levels were from 0.131 to 168 µg puff−1.

Although advertising may make it look convenient and appealing, it’s important to know that these products deliver varying amounts of the addictive chemical nicotine, which can negatively impact your learning, attention span and proneness to addiction. Many teens are taking things a step further, adding cannabis, CBD oils and other dangerous additives to vaping devices. When patients show up to the emergency department in respiratory distress from vaping, it can be challenging for physicians to treat them due to the difficulty in correctly identifying what they inhaled, especially when they are intubated or unconscious. All this evidence would suggest that e-cigarettes are potentially less harmful than conventional cigarettes (Fig. 2) [11, 14, 22, 24, 27,28,29]. Importantly, however, most of these studies have investigated only short-term effects [10, 14, 15, 22, 27,28,29, 31, 32], and the long-term effects of e-cigarette consumption on human health are still unclear and require further study. Vapes come in colorful, fun, and unique product designs, along with many flavors, all of which come together as part of the attraction for youth who believe that these products are not addictive and are safe to use. Many users believe vaping will reduce stress, anxiety, and depression.
