Separate studies have shown that newer e-cigarettes may deliver higher doses of nicotine more efficiently, which could explain the recent findings. The investigators acknowledged that more frequent use of e-cigarettes may also reflect greater determination to quit smoking combustible cigarettes. They noted that their study did not evaluate the risks of youth e-cigarette use. E-cigarettes are the most commonly used commercial tobacco product among youth,1 so it is critical that public health officials and the general public understand the potential risks of using them.
This was a large increase from 2019, when only 11.3% of people had ever used e‑cigarettes, and 2.5% were currently using them (Figure 1). The proportion of people who used e‑cigarettes daily in 2022–2023 was 3.5%, rising from 1.1% in 2019. In smokers who smoke tobacco cigarettes, blood flow increased modestly after cigarette inhalation and then decreased with subsequent stress. However, with those who vaped, blood flow decreased after inhalation at rest and also after handgrip stress. Finally, lung health effects are not expected to be seen for 20 to 30 years.
Guidance exists on best practices on this concept of accountability — known as end-producer responsibility — but isn’t enforced across the industry by any governing body. In many e-cigarettes, puffing activates the battery-powered heating device, which vaporizes the liquid in the cartridge. The person then inhales the resulting aerosol or vapor (called vaping).
At that point, levels of formaldehyde exceeded limits set by the California Office of Environmental Health Hazard Assessment (OEHHA), though other compounds didn’t come close. About a quarter of middle and high school students surveyed in 2017 said they had been around someone vaping at least once in the past 30 days. In 2021, only about 11 percent of high school students and 3 percent of middle school students surveyed said they had tried e-cigarettes. Results from a clinical trial published by JAMA Internal Medicine found that young adults ages who used This is Quitting from Truth had nearly 40% higher odds of quitting compared to a control group.
As such, it is imperative that e-cigarette cessation programs focused on adolescents are developed, evaluated, and implemented," Halpern-Felsher wrote. "This may point to the changing norms around e-cigarette use and the fact that quitting vaping among young people is becoming more normative and accepted," she said. Most of the study participants, about 87%, reported that they had tried to quit in the previous year, and about 94% reported feeling somewhat or very addicted to vaping. E-cigarettes usage by adolescents has become an epidemic according to the American Lung Association–20% (5 million) of all youth use e-cigarettes, a 135% increase in just two years.
And nicotine use in young adults still can lead to other illicit substance use. The request from the Federal Trade Commission asks six companies to turn over data by January dealing with the sale and promotion of their products for the years 2015 to 2018. In the United States, states are a primary determinant of the total tax rate on cigarettes. As part of the Family Smoking Prevention and Tobacco Control Act, the federal government collects user fees to fund Food and Drug Administration (FDA) regulatory measures over tobacco.
These findings can reflect general openness to nicotine use, indicated by a previous study showing considerable overlap between susceptibility types [24]. Several school-level factors have a role in both youth tobacco use and educational pathways. These include school rules and policies, physical environment, curriculum, discipline, school health services, school social and learning environment, student commitment to learning and school community as well as parents and the broader community [26]. Further, school influence partly occurs through the perceptions of smoking behaviour in the social network (descriptive norms) and perceptions of social network beliefs (injunctive norms), which both predict smoking initiation among youth [27].

Here we address some of the common questions people ask about e-cigarettes. ENDS may be manufactured to look like conventional combusted cigarettes, cigars, or pipes. Larger devices, such as tank systems or mods, bear little or no resemblance to cigarettes.
The ACS encourages young people currently using any of these products to ask for help in quitting and to quit as soon as possible. E-cigarettes can be especially harmful for young people because nicotine affects them in different ways than adults. People who use e-cigarettes should make sure they do not vape around children and ensure they always store e-cigarettes and e-liquids out of reach of children when not in use.
Vape products are taxed at the sales tax rate rather than the tobacco or cigarette tax rate. While vape products are not covered by the Smoke-free Air Act, each business can prohibit the use of vape devices indoors, but this is not required by state law. Iowa law places several restrictions on youth access to e-cigarettes, including but not limited to, prohibiting the sale, distribution, possession, purchase and use of vapor products to anyone under the age of 21. There are health and safety concerns related to the use of vapor products.

This study is the first published randomized clinical trial of a quit vaping program among teens, filling an important gap in the scientific literature and providing much needed evidence for pediatricians, school administrators, and public health agencies. Although stopping smoking can cause short-term side effects such as reversible weight gain, smoking cessation services and activities are cost-effective because of the positive health benefits. Further, educational aspirations and school engagement are tied to student composition and school-based networks and norms, which have had different associations with smoking between schools [28].
In 2021 "other" flavored devices made up 71 percent of all disposable devices sold or given away, with the most-popular subcategories being fruit-flavored and fruit & menthol/mint flavored products. These two subcategories alone made up more than half of all disposable e-cigarette devices sold or given away in 2021. Researchers said that the study’s prospective observational design allows them to infer, but not conclusively determine, a causal relationship between e-cigarette use and heart failure. However, with its large sample size and detailed data on substance use and health information, Bene-Alhasan said the study is one of the most comprehensive studies to assess this relationship to date. In fact, they are widely used as alimentary and pharmaceutical products [2]. In an analysis of 54 commercially available e-liquids, PG and glycerol were detected in almost all samples at concentrations ranging from 0.4% to 98% (average 57%) and from 0.3% to 95% (average 37%), respectively [35]. According to a 2018 study examining infrequent cannabis smoking in adults, vaping THC produced stronger mind-altering effects than smoking a similar amount of weed. However, nicotine alone is relatively harmless, and switching from daily tobacco smoking to daily e-cigarette use can be an important step for people to stop smoking completely. However, researchers found that only daily e-cigarette use had a statistically significant effect on smoking cessation rates. Research generally accepts that while vaping can harm the lungs and other bodily systems, its impact is much less than tobacco smoking.
Based on the current literature, it seems that several factors have led to the success of e-cigarette use as a smoking cessation tool. First, some e-cigarette flavours positively affect smoking cessation outcomes among smokers [102]. Second, e-cigarettes have been described to improve smoking cessation rate only among highly-dependent smokers and not among conventional smokers, suggesting that the individual degree of nicotine dependence plays an important role in this process [97]. Third, the general belief of their relative harmfulness to consumers' health compared with conventional combustible tobacco [103]. And finally, the exposure to point-of-sale marketing of e-cigarette has also been identified to affect the smoking cessation success [96].
Given their relatively recent introduction, there has been little time for a scientific body of evidence to develop on the health effects of e-cigarettes. Notably, the reverse was not true—students who said they smoked cigarettes were no more likely to report use of e-cigarettes when asked approximately 6 months later. Electronic cigarettes, or e-cigarettes, are tobacco products that have been sold in the U.S. for about a decade. They include e-pens, e-pipes, e-hookah, and e-cigars, known collectively as ENDS—electronic nicotine delivery systems. They're also sometimes called JUULs, "vapes" and "vape pens." E-cigarettes are the most commonly used tobacco products among kids—and it's become an epidemic. While much remains to be determined about the lasting health consequences of e-cigarettes, there’s evolving evidence about the health risks of e-cigarettes on the lungs—including irreversible lung damage and lung disease.

However, the state's largely rural landscape and small population could be partly to blame for its high youth vaping rate. About 80 percent of Wyoming's residents live in rural areas, as much of the land area is used for farming and national parks. Researchers combed through state-by-state data from the Campaign for Tobacco-Free Kids, an advocacy group aimed at stopping youth smoking. Since 2020, emergency department visits in Virginia by patients who said they vape jumped significantly, while visits by those who use traditional tobacco declined, according to stats from Virginia Hospital & Healthcare Association.
A Yale study in 2019 found that, among students at three Connecticut public schools, those who used e-cigarettes were more likely to smoke regular cigarettes in the future. Most e-cigarettes have a battery, a heating element, and place to hold a liquid (such as a cartridge or pod). The e-liquid usually contains nicotine, flavorings, and other chemicals. As the user draws on the device, the battery heats the e-liquid to produce aerosol (not water vapor).
Currently, e-cigarettes are not mentioned in the Illinois Smoke-free Illinois Act, which prohibits smoking in virtually all public places and workplaces, but some local ordinances do include e-cigarettes. E-cigarette aerosol is not harmless; it can contain harmful and potentially harmful substances, including nicotine and cancer-causing chemicals. While e-cigarettes typically have fewer chemicals than regular cigarettes, researchers found e-cigarettes present their own unique health risks and can increase the odds of chronic cough, phlegm, bronchitis, and asthma. Some cigarette smokers have replaced traditional smoking with airplane electronic cigarette cigarettes (e-cigarettes) or vaping, thinking it is not as dangerous. There are significant negative health effects from smoking cigarettes such as cancer, chronic obstructive pulmonary disease (COPD), heart disease, birth defects, and other health problems relating to nearly every organ of the body.
The researchers said that the findings suggest that if tobacco cigarette smokers who use e-cigarettes for a healthier alternative to tobacco won’t benefit by the switch. Cannabis and CBD e-liquids usually contain other chemicals, such as base liquids or flavoring agents. They may cause side effects similar to those of nicotine-free e-cigarettes. Young people who vape nicotine are more likely to start smoking cigarettes in the future. The 2018 NAP report concluded there’s some evidence that nicotine and nicotine-free e-cigarettes can damage oral cells and tissues in people who don’t smoke cigarettes.
In Cumberland County, Chalmers McClain and Graham said the growing number of vape shops opening near schools with their colorful and bright lights to attract students is also part of the problem. For example, Ramsey Street Alternative High School has two vape shops within a six-minute walk from the school. Teacher concerns around increasing the suspension rates of students caught vaping on school property prompted the initial pilot and current implementation of CATCH My Breath into county schools, said Graham.
The decision to pursue a smoking cessation objective, even in such a controlled form, should be made only after considering national circumstances, along with the risk of uptake and after exhausting other proven cessation strategies. Both smoking and vaping involve heating a substance and inhaling the resulting fumes. With vaping, a device (typically a vape pen or a mod — an enhanced vape pen — that may look like a flash drive) heats up a liquid (called vape juice or e-liquid) until it turns into a vapor that you inhale. Electronic smoking devices or e-cigarettes are battery operated devices used to inhale a vaporized liquid solution that frequently contain nicotine, flavorings and other chemicals. Because the liquid solution is converted into an aerosol vapor, e-cigarette use is often referred to as "vaping," rather than smoking. E-cigarettes are devices that heat a liquid, called e-liquid, into an aerosol that the user can inhale.
However, it has been reported that the heating process can lead to the generation of new decomposition compounds that may be hazardous [4, 5]. The levels of nicotine, which is the key addictive component of tobacco, can also vary between the commercially available e-liquids, and even nicotine-free options are available. For this particular reason, e-cigarettes are often viewed as a smoking cessation tool, given that those with nicotine can prevent smoking craving, yet this idea has not been fully demonstrated [2, 6, 7].
E-cigarettes are not safe for youth, young adults, and pregnant women, and can be dangerous for adults who use tobacco products. The novel results for school-level factors indicate that prevention of nicotine use may be supported by facilitating nicotine-free schools through policies and programmes. This may address especially the peer selection processes, which have been observed both in school disengagement and smoking initiation [28, 29]. Further, it may influence the students’ descriptive and injunctive norms, which both predict smoking initiation among youth [27].
Though not all flavors have yet been scientifically studied, there is a strong chance others may also be harmful. We work with community partners to raise awareness of the harms of e-cigarettes and vaping and flavored tobacco products that attract youth, through the Unhyped and CounterBalance campaigns. We also review and share e-cigarette use data and trends and tailor our education efforts accordingly. Youth ENDS use raises concerns about nicotine addiction, negative effects of nicotine on adolescent brain development, and other potential health harms, including increased risk of initiating cigarette smoking. Electronic nicotine delivery systems (ENDS), also called electronic cigarettes, e-cigarettes, vaping devices, or vape pens, are battery-powered devices used to smoke or "vape" a flavored or unflavored solution which usually contains nicotine. The American Academy of Family Physicians (AAFP) recognizes the increased use of ENDS, especially among youth and young adults, as well as its use by those attempting to quit smoking tobacco. Although e-cigarettes do not contain tobacco, for regulatory purposes, they are considered "tobacco products" by the US Food and Drug Administration (FDA) and the Centers for Disease Control and Prevention (CDC). E-cigarettes can go by different names, including vapes, vape pens or sticks, e-hookahs, hookah sticks, mods, and personal vaporizers.
Cigarette smoking is still the No. 1 cause of preventable death in this country, killing nearly five hundred thousand people each year. (According to some studies, more than half of longtime smokers will die from smoking-related complications.) It’s incredibly hard to stop smoking; people spend lifetimes trying. Around seventy per cent of American smokers say that they want to quit, and for many of them e-cigarettes have been a godsend. But, according to a 2017 study by the C.D.C., about fifty per cent more high schoolers and middle schoolers vape than smoke. Young people have taken a technology that was supposed to help grownups stop smoking and invented a new kind of bad habit, one that they have molded in their own image. The potential public-health benefit of the e-cigarette is being eclipsed by the unsettling prospect of a generation of children who may really love to vape.
These devices are permitted on planes, but the FAA recently notified airlines that the lithium batteries used in these devices are fire hazards and should not be packed in checked baggage. Dr Goel praised the central government’s initiatives to discourage tobacco consumption, including establishment of over 429 Tobacco Cessation Centers (TCCs), awareness campaigns, and the National Tobacco Quit Line. While the federal purchasing age of tobacco is 21, North Carolina law still lists it as 18, and many vape shops are operating under that age.
While e-cigarette use prevalence has declined significantly since the heyday of JUUL — 10% of high school students reported using e-cigarettes in 2023 — e-cigarette use among young people remains a concern. Thousands of flavored, high-nicotine, and relatively cheap e-cigarette products remain on the market — many of them illegally — driving youth use and nicotine addiction. Equally concerning, nearly half of young people who have ever tried e-cigarettes continue to use them, and many do so daily. As encouraging as the data was a few years ago, it’s starting to look like that’s not the case. The FDA is yet to approve them as a smoking cessation aid and a recent CDC study found that most adult e-cigarette users — 58.8 percent of them — don't stop smoking cigarettes and instead wind up using both products. The few scientists actively trying to fill the gap in the research literature are running into obstacles.
In many high and middle income countries lower socioeconomic status is a strong predictor of smoking. In North America, the common name for the remains of a cigarette after smoking is a cigarette butt. In Britain, it is also called a fag-end or a dog-end.[44] The butt is typically about 30% of the cigarette's original length. It consists of a tissue tube which holds a filter and some remains of tobacco mixed with ash. According to data from the World Health Organization,[39] the amount of tobacco per 1000 cigarettes fell from 1.03 kg (2.28 pounds) in 1960 to 0.41 kg (0.91 pounds) in 1999, largely as a result of reconstituting tobacco, fluffing, and additives. The epidemic use of e-cigarettes is a complicated problem which requires a cooperative effort between partners, organizations, and communities.
Some people believe that e-cigarettes can help them quit smoking tobacco. However, nicotine patches and many other FDA-approved quit aids are available now to help people quit smoking. E-cigarettes are not currently approved by the FDA as aids to help stop smoking.
The decision lends new credibility to vaping companies’ longstanding claim that their products can help blunt the toll of smoking, which is blamed for 480,000 U.S. deaths annually due to cancer, lung disease and heart disease. E-cigarettes have been sold in the U.S. since 2007 but in recent years their potential benefits for smokers have been overshadowed by their use by adolescents and teens. Vaping nicotine has not been thoroughly evaluated in scientific studies. For now, not enough data exists on the safety of e-cigarettes, how the health effects compare to traditional cigarettes, and if they are helpful for people trying to quit smoking.
E-Cigarettes, also called personal vaporizers or "vapes", present another way for smokers to ingest nicotine. E-Cigarettes have been marketed to young adults and adolescents through the use of candy and fruit flavors. They are also touted by some users as a "safer" alternative to smoking, and as a way to either quit smoking cigarettes, or to smoke in places where cigarette smoking is not allowed carry electronic jetblue cigarettes on a plane.
Screen all patients, encourage them to quit, and refer patients to evidence-based services for help quitting. However, nicotine used in vaping liquids is often sourced and extracted from tobacco plants. More recently, products containing synthetic nicotine, which is created through chemical processes, have been made available in Canada.
