The Centers for Disease Control and Prevention reports that nicotine can also harm parts of the brain that control attention, mood and impulse control. Electronic Nicotine Delivery Systems (ENDS) represent a rapidly changing class of tobacco products known by many different names, including e-cigarettes, e-cigs, vapes, mods, and tank systems. ENDS deliver an aerosol to the user that typically contains nicotine, propylene glycol, vegetable glycerin and flavoring chemicals. In 2018, 3.2% of U.S. adults (age 18 and older) reported current ("every day" or "someday") use of ENDS. Since 2014, ENDS have been the most used tobacco product among U.S. youth. In 2020, about 3.6 million U.S. youth, including about 1 in 5 high school students and 1 in 20 middle school students, reported current (past 30 day) use of ENDS.
The "e-juice" that fills the cartridges usually contains nicotine (which is extracted from tobacco), propylene glycol, flavorings and other chemicals. E-cigarettes are sometimes called e-cigs, vapes, vape pens, e-hookahs, and electronic nicotine delivery systems or ENDS.

N.L.B. serves as a consultant to pharmaceutical companies that market or are developing smoking cessation medications, and has provided expert testimony in litigation against tobacco companies. Despite this trend all current evidence finds that e-cigarettes carry a fraction of the risk of smoking. The 966 respondents who had never vaped more often believed that e-cigarette waste was dangerous to throw in trash compared to the 1,083 respondents who had vaped at least once (81.4% vs. 71%). Fewer of those who had vaped at least once (79.3%) believed that e-cigarettes contained toxic substances compared to those who had never vaped (89.6%). Young respondents clearly recognized the dangers of e-cigarette waste to humans and the environment and want an appropriate method to recycle.
While e-cigarette use prevalence has declined significantly since the heyday of JUUL — 10% of high school students reported using e-cigarettes in 2023 — e-cigarette use among young people remains a concern. Thousands of flavored, high-nicotine, and relatively cheap e-cigarette products remain on the market — many of them illegally — driving youth use and nicotine addiction. Equally concerning, nearly half of young people who have ever tried e-cigarettes continue to use them, and many do so daily. As encouraging as the data was a few years ago, it’s starting to look like that’s not the case. The FDA is yet to approve them as a smoking cessation aid and a recent CDC study found that most adult e-cigarette users — 58.8 percent of them — don't stop smoking cigarettes and instead wind up using both products. The few scientists actively trying to fill the gap in the research literature are running into obstacles.

Given their relatively recent introduction, there has been little time for a scientific body of evidence to develop on the health effects of e-cigarettes. Vape aerosols may also increase the risk of heart disease, lung cancer, and asthma complications. The substitution was especially evident among cigarette brands popular with young people aged 20 and under, suggesting that flavor restrictions may increase smoking among youth as well as adults. The immense popularity of electronic cigarettes, or e-cigarettes, among young people has led many policymakers to restrict the sale of flavored varieties. Proponents of e-cigarettes claim they’re safer than smoking because they don’t contain the more than 60 cancer-causing chemicals in tobacco smoke and are not combustible. But e-cigs still deliver harmful chemicals, including nicotine, the extremely addictive substance in cigarettes.

Carbonyls in cigarette smoke are formed mainly by pyrolysis of tobacco sugars83, whereas those in e-cigarettes are formed mainly by thermal degradation of PG and/or VG83,84,85. Flavourings may also contribute to the formation of carbonyls, as well as the characteristics of the e-cigarette devices, especially the applied voltage, coil resistance and wicking material47,48,49,86,87. Poor wicking efficiency may lead to a dry wick and overheated e-liquid (dry puff), which promotes the formation of carbonyls and other toxic compounds2,10,13,15. Coil location, orientation, resistance and wick material, as well as power output, have been shown to affect carbonyl generation significantly13,15,86.

In an analysis of 54 commercially available e-liquids, PG and glycerol were detected in almost all samples at concentrations ranging from 0.4% to 98% (average 57%) and from 0.3% to 95% (average 37%), respectively [35]. The Policy Scan Project, by the Institute for Global Tobacco Control (at Johns Hopkins University) tracks and reports regulatory approaches to e-cigarettes around the world. In 2012 and 2013 there was a flurry of tobacco company investment in e-cigarettes, both in the United Kingdom (UK) and in the United States (US), a trend which continued in subsequent years. In 2018, 8% of Pierce County 12th graders said they had smoked a cigarette in the past 30 days. Among people who were currently using e‑cigarettes, the proportion of people who used them daily remained stable from 2016 (42%) to 2019 (42%), then increased in 2022–2023 (49%). While more males than females used e‑cigarettes daily, the change among females was much greater than among males, rising from 36% in 2019 to 51% in 2022–2023. This corresponds to a fourfold increase in the number of females using e‑cigarettes daily, from approximately 70,000 to 300,000 (Figure 2).
Any product that does not submit an application by this deadline must be removed from the marketplace, which if properly enforced, could lead a significantly smaller marketplace. The main component of e-cigarettes is the e-liquid contained in cartridges or tanks. To create an e-liquid, nicotine is extracted from tobacco and mixed with a base (usually propylene glycol), and may also include flavorings, colorings and other chemicals (such as formaldehyde and acrolein, which can cause irreversible lung damage). The American Lung Association is very concerned that we are at risk of losing another generation to tobacco-caused diseases as the result of e-cigarettes.
Among other things, the law prohibits the sale of vapor products to people under 18 years of age, requires sellers to obtain a license, and requires that vapor products in stores are inaccessible to minors. E-cigarettes are hooking a new generation on nicotine – putting millions of kids at risk and threatening decades of progress in reducing youth tobacco use. It’s a nationwide crisis of youth addiction, fueled by thousands of kid-friendly flavors and massive doses of nicotine. While secondhand smoke from cigarettes causes serious harm to others, there is no evidence so far that vaping is harmful to people around you and any risks are likely to be very low.
The researchers said that the findings suggest that if tobacco cigarette smokers who use e-cigarettes for a healthier alternative to tobacco won’t benefit by the switch. Cannabis and CBD e-liquids usually contain other chemicals, such as base liquids or flavoring agents. They may cause side effects similar to those of nicotine-free e-cigarettes. Young people who vape nicotine are more likely to start smoking cigarettes in the future. The 2018 NAP report concluded there’s some evidence that nicotine and nicotine-free e-cigarettes can damage oral cells and tissues in people who don’t smoke cigarettes.

"If there was good evidence that people were using e-cigarettes just to quit smoking, there would be wide support," Benowitz says. "The problem is most of the e-cigarette use in the U.S. is dual use with cigarettes." People use e-cigs in places or situations where they can't smoke, like in a restaurant, but continue lighting up when they can, he explains. "My reading of the evidence is that it is quite convincing that e-cigarettes are helping some people quit smoking." Many young e-cigarette users reported throwing away, improperly recycling, or littering the devices. Only 15% of young e-cigarette users reported disposing of empty pods or disposable vapes by dropping them off or sending them for electronic recycling.
They also contain toxins that can damage your lungs and overall health. One of the reasons why the e-cigarette epidemic among young people is so troubling is that e-cigarette use seems to lead to the use of traditional cigarettes. This makes them a better choice for people who already smoke or use other tobacco products. Vaping may have some benefits for smokers who switch to it as a complete substitute for using other tobacco products. E-cigarette manufacturers are using marketing tactics used by the tobacco industry to lure kids to conventional cigarettes.
In this particular aspect, again the composition of the e-liquid varies significantly among different commercial brands [4, 35]. The most common and major components of e-liquids are PG or 1,2-propanediol, and glycerol or glycerine (propane-1,2,3-triol). Both types of compounds are used as humectants to prevent the e-liquid from drying out [2, 53] and are classified by the Food and Drug Administration (FDA) as "Generally Recognised as Safe" [54].
A 2016 study found that people who use or have used e-cigarettes are less likely to stop smoking altogether. Flavors vary widely, from "traditional" and menthol to watermelon and "lava flow." Some e-cigarettes taste like traditional cigarettes and even mimic the tastes of specific brands. When the user sucks on the mouthpiece, the heating element vaporizes the solution, which the person then "vapes," or inhales. The nicotine content of the liquid can range from "very high" to zero. Refills come in flavors such as cool cucumber, mango, and mint, which may seem natural and harmless, but a single JUUL refill contains as much nicotine as a pack of 20 cigarettes. To inform this work and evaluate the effectiveness of policy, the CDC Foundation is collecting and analyzing information about e-cigarette sales, teen e-cigarette use, and teens’ attitudes about the epidemic.
One chemical, known as 6-methyl nicotine, has been shown in rodent experiments to be far more potent than nicotine in targeting the brain’s nicotine receptors and more toxic than nicotine. Smoking harms nearly every organ in the body ― heart and blood vessel disease; lung disease; cancer (almost anywhere in your body); and impacts to your bones, eyes, teeth, gums, fertility and pregnancy. Individual businesses can prohibit the use of vape devices indoors but this is not required by state law. More adults 18 to 24 years old used e-cigarettes (11.0%, or roughly 3.4 million people) compared with adults 25 to 44 (6.5%) or 45 and older (2.0%). The latest research, published in a July National Center for Health Statistics data brief, shows that in 2021, e-cigarette use was highest among adults 18 to 24 years old.
The increasing number of adolescent users and reported deaths in the United States prompted the government to ban the sale of flavoured e-cigarettes in 2020. The difference in opinion worldwide may be due to different restrictions imposed. For example, while no more than 20 ng/mL of nicotine is allowed in the EU, e-liquids with 59 mg/dL are currently available in the United States. Nevertheless, despite the national restrictions, users can easily access foreign or even counterfeit products online. Is there sufficient toxicological data on all the components employed in e-liquids?
However, within the adult demographic, 12.7 percent of 18- to 24-year-old adults and 21.3 percent of males, ages 18-24, are "current users" of e-cigarettes. The e-cigarette device (Vype ePod1.0, Nicoventures Trading Ltd., Blackburn, UK) consists of a metallic outer device case, a printed circuit board to control the device, a lithium-ion rechargeable battery (350 mAh) and an e-cigarette cartridge (Fig. 1). The voltage ranges from 2.2 to 3.1 V and is not adjustable by the user. The device has dimensions (h × w × d) of 104.2 × 19.1 × 10.5 mm and a power output of 6.5 ± 0.5 W. The cartridges or pods consist of a plastic case holding the ceramic wick material and a flat metal heating element (NiCr, 0.8 –1.4-Ω resistance). Each pod is pre-filled with Vype e-liquid (1.9 mL) and is magnetically attached to the device.
Yet another analysis linked e-cig use with greater odds of trying tobacco. They come in kid-friendly flavors, including chocolate, bubble gum and gummy bear. Sold online and in the mall, e-cigarettes are also easy for minors to acquire.
That’s about twice as many people as those who used other methods to quit smoking. Of people who used other methods, 91% kicked nicotine products altogether. People usually think vaping isn’t as bad as cigarette smoking, but the mist you breathe in still has nicotine and other harmful chemicals in it. Vaping isn’t safe and can cause health problems, including life-threatening lung injuries.

Little research has been conducted into the safety of e-cigarettes and e-liquids in pregnancy. It is not known whether the vapour is harmful to a baby in pregnancy. E-cigarettes do not produce tar or carbon monoxide, two of the most harmful elements in tobacco smoke.
As with high school students, e-cigarette use was much more common than cigarette use. E-cigarettes come in many shapes and sizes and can also be called e-cigs, e-hookahs, vapes, vape pens, personal diffusers or diffuser sticks. But one vape can contain the equivalent of 50 cigarettes of nicotine in the one product. They also contain a complex combination of chemicals including nicotine, nickel, tin, arsenic, chlorine and lead. These substances can be harmful to the body, with different flavourings helping to mask these poisons when they are taken into the body. An electronic cigarette (e-cigarette) is a battery-operated device that heats liquid nicotine, flavoring, and other additives to create an aerosol.
According to the CDC, 1 in 4 deaths in the U.S. result from cardiovascular disease caused by cigarette smoking. Teens who smoke can end up with lungs that are smaller and weaker than the lungs of teens who don’t smoke. Damage to the respiratory system can also make you more susceptible to certain infections that affect the lungs, like tuberculosis and pneumonia, and increase the possibility of death from those illnesses.
The composition of e-liquids requires stricter regulation, as they can be easily bought online and many incidences of mislabelling have been detected, which can seriously affect consumers’ health. Beyond their unknown long-term effects on human health, the extended list of appealing flavours available seems to attract new "never-smokers", which is especially worrying among young users. Additionally, there is still a lack of evidence of e-cigarette consumption as a smoking cessation method. Indeed, e-cigarettes containing nicotine may relieve the craving for smoking, but not the conventional cigarette smoking habit.
So, if you are vaping to quit smoking, you should aim to eventually stop vaping too. In 2022, UK experts reviewed the international evidence and found that "in the short and medium term, vaping poses a small fraction of the risks of smoking". Vapes are electronic devices that let you inhale nicotine in an aerosol, or vapour, instead of smoke. This is done by heating a solution (e-liquid) that typically contains propylene glycol, vegetable glycerine, flavourings and nicotine. Vapes come in many shapes and sizes but they have the same basic components, including a battery, sensor, and atomizer/ flavor cartridge.
The term "vaping" comes from the act of inhaling and exhaling e-cigarette vapor (which is actually an aerosol). E-cigarettes heat up liquid from replaceable cartridges, producing the vapor. Just like a smoker does, an e-cigarette user inhales the vapor through the device, then exhales. People who switch from traditional cigarettes to e-products may put off getting medical help or trying proven tools that can help with quitting. United States federal law does not allow the sale of tobacco products to people under the age of 21.
Median odds ratio (MOR), which shows the difference between two respondents with identical student-level characteristics from two randomly chosen different schools, was calculated to express between-school variance on the odds ratio scale [36]. The analysis was not pre-registered, and the results should be considered exploratory. This study was the first to use endothelial cells derived from iPS cells to directly investigate the effect of e-liquids with and without nicotine on their viability and function.
NSW Health has launched a new mass media behaviour change campaign aiming to address the issue of vaping by young people. The campaign, ‘Every vape is a hit to your health’ utilises powerful creative to encourage young people to consider the proven health harms of vaping, such as nicotine addiction, lung damage, breathlessness, nicotine poisoning and burns from exploding vapes.
